Support Worker
Duties:
- Provide support and assistance to individuals with physical or intellectual disabilities
- Assist with daily activities such as personal care, meal preparation, and medication management
- Support individuals in maintaining a clean and safe living environment
- Promote independence and encourage individuals to participate in community activities
- Document and report any changes in behavior or health status to the appropriate supervisor
- Collaborate with a multidisciplinary team to develop and implement individualized care plans
