Are you passionate about making a positive difference in the lives of individuals with mental health needs? We are seeking a dedicated and compassionate Social Care Coordinator to join our Mental Health Team within Adult Social Care. We also have a focus on re-enablement.

About the Role:
We work with people with complex mental health difficulties, people with forensic histories, carers and families, and we have a re-enablement service and hoarding service within our team. We seek creative ways to work together to meet the needs of people who use our services together.

The Mental Health Social Care Team aims to actively promote wellbeing and independence, working with people with complex mental health difficulties in partnership with health colleagues. Supporting people to retain or regain their skills and confidence by using existing and new support networks, Technology Enabled Care (TEC), equipment and short-term interventions.

Key Responsibilities:
  • Provide high-quality support and coordination of care services for adults with mental health needs.
  • Work collaboratively with service users, families, carers, and multi-disciplinary teams to develop and implement personalised care plans.
  • Assess, monitor, and review the needs of individuals to ensure appropriate services are provided.
  • Advocate for service users, ensuring their voice is heard and their rights are respected.
  • Maintain accurate records and reports in line with legal and organisational requirements.
  • Working to undertake assessments, provide care plans and meet the needs of adults with mental heath needs.
  • To undertake reviews and make amendments to care plans of adults with mental health needs.
  • Experience working with people with mental health difficulties and or complex needs.

You will hold a caseload of people with mental health difficulties and due the nature of the work you will need to have a good understanding of risk management; managing yourself in difficult environments; flexible and able to analyse varied information or situations to produce solutions.

You will work within the legal framework provided by the statute, guidance, policies, and procedures.

This role offers flexible working arrangements, including remote working options. However, applicants must be within commuting distance of Reading as site visits are an essential part of the role.
