Key Responsibilities
- Client Assessment: Evaluate clients' needs and develop personalized care plans in collaboration with healthcare professionals and family members.
- Service Coordination: Schedule and coordinate caregiver visits, ensuring that all services are delivered effectively and efficiently.
- Monitoring and Quality Assurance: Regularly review and monitor the quality of care provided, addressing any issues or concerns that arise.
- Communication: Serve as the primary point of contact for clients and their families, providing updates and addressing questions or concerns.
- Documentation: Maintain accurate records of client care, progress, and any changes in health status, ensuring compliance with regulatory requirements.
- Staff Management: Supervise and support home health aides and other caregivers, providing guidance and training as needed.
- Administrative Duties: This role often includes administrative tasks such as maintaining patient records, managing care documentation, and ensuring compliance with healthcare regulations
- Skills: Excellent communication, organizational, and interpersonal skills are essential. A compassionate and patient demeanour is also important for this role.
- Work Environment: Home Care Coordinators typically work in an office setting but will frequently travel to clients' homes to assess needs and monitor care. Flexibility in working hours may be required to accommodate client schedule
- To ensure compliance with all office systems and procedures.
- Assistance with wages and invoice processing.

- Ability to induct and orientate new employees to the job role and service
- Organise and plan supervision and appraisals for care staff
- Provide and oversee staff providing service users with support/assistance as they accomplish daily tasks, including bathing, eating, dressing, grooming and using the bathroom
- Ensure that the staff allocated and the staff attending each service user have the relevant skills, knowledge and resources related to the opportunities and needs of that person
- Communicate any problems, concerns or changes to peoples’ family members as needed
- Knowledge of rota planning, ensuring adequate staff on duty with correct skill mix and fair allocation of off duty/annual leave
- Understanding of legal responsibilities and standards for CQC registered services, including person-centred service delivery.
- Must have UK Driver's Licence
Experience
- Previous experience in an office environment is essential, with a focus on administrative or coordination roles preferred.
- Familiarity with healthcare systems or experience in a caregiving role would be advantageous.
- Strong communication skills, both verbal and written, are crucial for effective interaction with clients and healthcare professionals.
- Demonstrated ability to manage multiple tasks efficiently while maintaining attention to detail is required.
- A compassionate attitude towards clients and their families is essential for success in this role.
