Primacy Homecare Ltd is a domiciliary care company based in Ickenham, covering the areas of Denham, Gerrard's Cross, Harefield, Harrow, Hillingdon, Ickenham, and Ruislip.

Key Responsibilities:
- Client Assessment: Conduct comprehensive assessments of clients' needs, preferences, and care requirements to develop tailored care plans.
- Care Coordination: Coordinate and schedule care services for clients, ensuring that the right caregiver is matched to each client based on their unique needs and preferences.
- Client Support: Provide ongoing support to clients and their families, addressing any concerns or changes in care needs promptly and effectively.
- Quality Assurance: Monitor the quality of care services provided, conducting regular reviews, and making necessary adjustments to ensure optimal care delivery.
- Team Collaboration: Collaborate closely with caregivers, nurses, and other healthcare professionals to ensure that care plans are followed accurately.
- Documentation: Maintain accurate and up-to-date client records, documenting all care activities and changes in care plans.
- Compliance: Ensure compliance with all relevant regulations, standards, and company policies.
- Client Advocacy: Act as an advocate for clients, ensuring that their voices are heard and their needs are met with compassion and respect.
Qualifications and Experience:
- NVQ Level 3 in Health and Social Care.
- Driving licence and access to own car.
- Previous experience in a similar role within the domiciliary care sector is highly desirable.
- Knowledge of care assessments and care planning.
- Strong organizational and communication skills.
- Empathetic and compassionate approach to client care.
- Ability to work effectively in a fast-paced and dynamic environment.
- Familiarity with relevant care regulations and best practices.
