Senior Ageing Well Co-ordinator x 1 County-wide 15 hours per week

About the Role
As a Senior Ageing Well Coordinator, you will supervise and support our team of Ageing Well coordinators who form part of a multidisciplinary team, working closely with GP surgeries and community partners with Primary Care Networks (PCN).
Your role will involve:
- The supervision, support and development of our Ageing well team
- Supporting our Ageing well team around case management and home visits
- Supporting our Ageing well team to carrying out holistic assessments and patient observations
- Representing Northamptonshire Carers within the Ageing Well teams
- Supporting Staff to help individuals avoid unnecessary hospital admissions or institutional care
- Supporting staff in promoting independence and wellbeing with people aged 65+ with complex long-term conditions —helping people choose well, stay well, and live well in their own homes

About You
We’re looking for someone who is:
- Experienced in Senior support work, Senior wellbeing coordination or within health and social care
- Experienced in the supervision, support and development of staff.
- Skilled in working collaboratively across multi-disciplinary teams
- Confident using electronic recording systems
- Knowledgeable about services available to support older people
- A great communicator—both written and verbal
- Empathetic, a good listener, and able to build trust with individuals and families
- Organised, self-motivated, and capable of managing a varied caseload independently
You may already be working as a primary care practitioner, within pcn primary care network or support worker
