About the Role

The role of a Health and Wellbeing Co-ordinator encompasses promoting health and wellbeing, fostering liaison and partnership, and maintaining effective monitoring systems. This involves conducting collaborative risk and needs assessments, offering time-limited interventions focusing on psychological education, and advocating for psychologically informed environments. Additionally, the Health and Wellbeing Co-ordinator identifies and mitigates risks, supports clients in transitioning out of support accommodation, and facilitates their integration into the local community.

The Health and Wellbeing Co-ordinator necessitates a holistic approach, emphasising collaboration, risk management, and continuous monitoring to support individuals experiencing homelessness in achieving health, wellbeing, and integration into the community.

This role is fixed term until 31st March 2026.
