Job Introduction
Based within our community Bradford Enablement Support Team which provides specialist enablement services to vulnerable people with assessed needs.We are looking for Community Enablement Coordinators to join our dedicated team in the BEST. This role is an opportunity to make a real difference in our vibrant and diverse city.
The main responsibilities of this role:
- Completing strength based assessments, in the community
- Working in partnership with Health and wider social care communities
- You will supervise and support a team of staff to ensure that the front line of the service provided by the council is delivered in accordance with all the relevant Departmental guidelines, policies and national care standards in partnership with other health and social care colleagues.
- Ensure records are of a high quality standard for individuals outcomes
- To attend and contribute to any relevant meetings
