We are seeking dedicated and compassionate Hub Enablement Workers join our team at Broxtowe Community Hub. You will be responsible for delivering support and activities to help people met their outcomes and provide personal care to individuals. Flexibility is required and the ability to work across buildings within Nottinghamshire.

This role is crucial in ensuring that we are providing exceptional day opportunities and support within our community hubs, specialist services, and within the community. Our mission is to enhance the quality of life for the people through innovative and compassionate care.

Hub Enablement Workers provide a range of assistance and support in response to the day-to-day personal, physical, and emotional needs of individuals, always respecting their dignity and being sensitive to age, disability, sexuality, gender, and cultural origin as described in care plans.

The role means you would support individuals within a risk management framework and escalate any risks or concerns to management. Hub Enablement Workers contribute to the completion of care plans based on effective consultation with individuals, carers, specialist workers, and other relevant agencies.

Hub Enablement Workers comply with departmental health and safety policies and understand their individual responsibilities regarding health and safety requirements and follow departmental practice guidance, policies, and procedures. Hub Enablement Workers deliver a wide range of day-to-day activities and support in accordance with care plans and the quality assurance framework and may work with individuals on a one-to-one basis or within groups both within a base or out in the community.

Hub Enablement Worker support individuals to promote and maintaining their independence and also contribute to the ongoing review and monitoring of care plans and risk assessments. There is a requirement to support with personal care and use equipment as directed, following training. Hub Enablement Workers respond appropriately to crisis and emergency situations and report any incidents as appropriate. There is an expectation that Hub Enablement Workers ensure confidentiality of information in respect of records maintained and tasks undertaken within NCC policy and relevant legislation.

You must:

  • Have a Professional care qualification or equivalent, or willing to work towards standards equivalent to Level 2 Diploma in Adult Care and the Care Certificate.
  • Have ICT skills.
  • Have excellent communication skills.
  • Have the ability to work efficiently and effectively, both independently and as part of a team.
  • Have a high level of punctuality and attendance.
  • Have experience of working directly with adults, individuals, or groups.
