About Us:
We are based in knott end on sea, 20 mins from Lancaster, 15 mins from Poulton le fylde. We are a small, family-run complex care company supporting clients with long-term health conditions in the comfort of their own homes. With a focus on compassion, communication, and high-quality care, we aim to support not just our clients, but their families too. We currently support two clients, with plans for growth.
We are looking for a Care Coordinator / Senior Care Lead to join our team — someone who is just as confident in the office as they are delivering care in the community.
The Role:
This is a hybrid role, combining care delivery with office-based coordination duties. You will play a key role in supporting the day-to-day running of our care service, ensuring both clients and carers receive the support they need.
You’ll be involved in rotas, onboarding, medication audits, staff training, client activities, and more — while also working shifts where needed, so you stay connected to the care you coordinate.
Key Responsibilities: Care Duties:
- Deliver personal, dignified, and responsive care to clients with complex needs.
- Administer medication and carry out clinical tasks when trained (e.g., PEG)
- Build strong, professional relationships with clients.
- Monitor client well-being and report any concerns.
Coordination / Office Duties:
- Create and manage staff rotas, responding to sickness or changes.
- Support with staff onboarding, induction, and sign-off on competencies.
- Carry out medication and care documentation audits.
- Organise meaningful activities for clients and encourage engagement.
- Be a key point of contact for carers, supporting with day-to-day queries and guidance.
- Maintain accurate records and communicate with health professionals as needed.
What We’re Looking For:
- Minimum NVQ Level 3 in Health & Social Care (or equivalent).
- Experience in complex care (PEG, epilepsy, catheter care, etc.).
- Confident working independently and leading small teams.
- Strong understanding of safe practice, safeguarding, and CQC standards.
- Comfortable using care systems, writing reports, and managing rotas.
- Full UK driving licence (Desirable)
- Willingness to work flexible hours including some evenings/weekends.
Job Type: Full-time
Pay: £15.00-£15.50 per hour
Expected hours: 37.5 per week
