Overview
The Care Coordinator plays a vital role in ensuring that clients receive comprehensive and coordinated care. This position involves working closely with carers, clients and their families to facilitate communication and streamline the care process. Responsibilities
- Serve as the primary point of contact for clients, families, and partnership organisations throughout the care provision.
- Coordinate rosters, referrals, and care to ensure continuity of services.
- Maintain accurate client records and documentation in compliance with CQC regulations.
- Collaborate with multidisciplinary teams to develop and implement individualised care plans.
- Monitor client progress and communicate updates to relevant stakeholders.
- Assist in resolving any issues or concerns related to client care promptly and efficiently.
Skills
- Strong organisational skills with the ability to manage multiple tasks simultaneously.
- Excellent communication skills, both verbal and written, to effectively liaise with clients and social and healthcare professionals.
- Proficient in rostering software applications for documentation and scheduling visits.
