Referrals Assessments and Move-On Coordinator
Safe Haven Sussex CIC provides shared accommodation and support to single vulnerable adults who present with support needs. The Referral and Move On role is vital to ensure that referrals and assessments are processed correctly and within the time scales set out in our service specifications. In addition to supporting our residents to move on to live independently.
You will be contacting and liaising with external referrers, the local authority, other community organisations and the clients. The clients will be disclosing very personal information, it is essential that we are committed to safeguarding their confidentiality and therefore it is vital that you adhere to all relevant legislation, policies and procedures including confidentiality and GDPR.
- Process all client referrals into Safe Haven Sussex
- Arrange and conduct initial ‘pre-assessment’ calls
- Arrange and conduct face-to-face housing assessments
- Coordinate the allocation of new residents to empty rooms
- Build positive relationships with stakeholders, current and prospective referrers to increase the volume of referrals to Safe Haven Sussex
- Advise referrers on the referral criteria and process
- Communicate with prospective service users, referrers and other stakeholders via phone and email.
- Update referrals, occupancy and empty rooms logs, and communicate clearly and promptly with staff.
- Provide support and advice to Support Coordinators regarding move on options and funding options to help with move on.
- Stay abreast of housing move on options
- Meet with service users who are considering moving on to give one-to-one advice and support.
- Co-deliver ‘renting ready’ tenancy skills course to small groups of residents who are looking to gain the skills to move on to independence.
