Modality Partnership Walsall Division has a full-time vacancy fixed term contract for 12 months for a PCN Social Prescriber. The postholder will work as part of a multi-disciplinary team within general practice at Kingfisher Berkley Practice within Modality Walsall Primary Care Network PCN. Main duties of the job
The role of the Social Prescribing Link Worker is to provide social support to patients across the PCN. You will work individually with patients, taking the time to listen and understand what matters to them, and supporting them to access additional services, groups or social activities.
The postholder will need to confidently evidence and utilise key skills such as effective communication, ability to empower people to take control of their health and wellbeing through referral to link workers who give time, focus on what matters to me and take a holistic approach to an individuals health and wellbeing, connecting people to diverse community groups and statutory services for practical and emotional support.
You will carry and manage a case load of patients, working independently within your remit to support patients. All social prescribers receive clinical supervision.
The postholder will work in collaboration with local divisions (including Divisional Boards) and the National Modality Business Support Team to deliver innovative ways of working to meet the strategic aims of the organisation.
The role is not a regular 9am-5pm job, the job requires innovation, flexibility, and commitment. The postholder will be required to work resourcefully as part of the team to ensure tasks are completed.

- Co-produce a simple personalised care and support plan to improve health and wellbeing, introducing or reconnecting people to appropriate community groups and statutory services.
- Manage and prioritise your own caseload, in accordance with the needs, priorities and any urgent support required by individuals on the caseload. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person’s needs are beyond the scope of the link worker role – e.g. when there is a mental health need requiring a qualified practitioner.

Person Specification Knowledge Essential
- NVQ Level 3, Advanced level or equivalent qualifications or working towards.
- Demonstrable commitment to professional and personal development.
- Training in motivational coaching and interviewing or equivalent experience (advantageous).
