PartnersHealth are seeking a highly motivated and organised individual with a commitment to delivering high-quality patient services to join our team as a Primary Care Network Team Leader. This role offers an excellent opportunity for a professional looking to contribute to the coordination and improvement of primary care services across Rushcliffe.
As a PCN Team leader you will provide high-quality leadership and line management to our Care Coordination team and support the smooth running of patient-facing services across our Rushcliffe practices.
About the Role
As PCN Team Leader, youll:
- Lead and manage the Care Coordination team across all Rushcliffe PCN practices
- Oversee the day-to-day operations of patient support services and ensure high standards of care and communication
- Provide operational and leadership support to practices, including acting as a Practice Team Leader when needed
- Support implementation of service changes and manage Enhanced Access provision along side the PCN Service Manager
- Liaise with internal and external stakeholders to improve services, resolve issues, and implement best practices

What Were Looking For
Essential:
- Previous team leadership or management experience, ideally in a healthcare setting
- Strong interpersonal and communication skills
- Excellent IT literacy (including Microsoft Office and ideally SystmOne)
- A proactive and flexible approach with the ability to prioritise and self-manage
- Full UK Driving Licence and access to a vehicle
Desirable:
- Management qualification
- Experience working in General Practice or Primary Care
- Knowledge of clinical audits, Quality and Outcomes Framework (QOF), and IIF indicators
