About the role: Porchlight is proud to be part of the evolving Kent & Medway mental health support landscape. As part of the Live Well Kent & Medway network, we’re delivering a new service (Living Better) that supports those with enduring mental health issues, as well as people with complex social factors that are affecting their personal recovery and ability to live independently with confidence
As a Mental Health & Housing Navigator, you’ll work one-to-one with clients to co-produce personalised recovery plans that reflect their goals, aspirations, and challenges. You’ll help people build resilience, improve mental health insight, and navigate housing issues. You’ll also work closely with a wide range of partners across health, social care, and the voluntary sector to ensure joined-up, effective support.
This is a community-based role that requires independence, initiative, and a strong commitment to recovery-focused practice. You’ll be part of a passionate and supportive team, helping to shape a service that truly meets people where they are.
The role requires:
- Experience supporting individuals with mental health and/or housing needs, ideally in a community setting
- Confidence in managing a caseload and recognising when to refer to other professionals or services
- A recovery-focused, strengths-based approach to support planning and delivery
- The ability to identify and manage risk and safeguarding concerns effectively
- Strong partnership working skills and the ability to build relationships with a range of local services
- Excellent record-keeping and the ability to maintain accurate, timely, and compliant client records
- A flexible, proactive, and person-centred mindset, with a commitment to continuous improvement and best practice
