About Us
One Wirral CIC is a not-for-profit organisation focused on reducing health inequalities by uniting health and community organisations to support local communities. We are partnering with Macmillan Cancer Support and Wirral University Teaching Hospital Trust to transform cancer support across Wirral, providing seamless, personalised care and support.
Role Overview
We are seeking a passionate, enthusiastic Macmillan Cancer Wellbeing Coordinator with experience in personalised care and support, partnership working and community engagement to work as part of this exciting new project. As a brand new service, this will involve a collaborative and flexible approach to working across organisations, sectors and stakeholders.
Working as part of an integrated team, the Macmillan Cancer Wellbeing Coordinator will be a key point of contact for people with cancer, providing a simple and efficient route to access support for their non-clinical holistic needs, facilitated by the offer of an eHNA and Personalised Care and Support Plan. These personalised conversations will take place in community settings, at a time and place that supports the individual best.
The Macmillan Cancer Wellbeing Coordinator will link in with teams in secondary and primary care settings and across the range of VCFSE (Voluntary, Community, Faith, Social Enterprise) organisations to upskill staff, raise awareness over the importance of personalised care and support for people with cancer.
Key Responsibilities
- Provide 1:1 community-based support to people with cancer and their families and carers – to help people to identify their needs. Usually by supporting them to complete a eHNA and then create a care plan with them to support those needs.
- Connect people to a range of support services to address issues that may be impacting on patient and carer wellbeing.
- Proactively build good working relationships and provide information and advice to a wide range of stakeholders.
- Upskilling VCFSE partner organisations to support with eHNA and care plans – working closely with the specific organisations for a period of time to embed the project model, principles and strategies.
- Liaise with the wider project team to feedback and work with the Macmillan Engagement Officer to actively seek patient involvement and better understand patients’ experiences.
- Promote personalised care within primary, secondary, and community settings.
- Support engagement events and opportunities
- Support service evaluation and contribute to project development.
What We’re Looking For
- NVQ Level 3/4 in Health and Social Care or equivalent experience.
- Experience of engaging with people, carers and service users with protected characteristics and/or from underrepresented groups.
- Knowledge of local VCFSE organisations and how they can work with the project.
- Strong communication and relationship-building skills.
- Ability to work independently and as part of a team.
- Empathy, approachability, and commitment to supporting people affected by cancer.
- Familiarity with the needs of people with cancer and an understanding of health inequalities.
There is 1 positions available of 22.5 hours (0.6 FTE)
