Delivery Programme – Connect to Work
Subject to contract award, Connect to Work will support Lancashire residents facing barriers to employment, including those with disabilities, health conditions or other challenges. This new government-funded programme, part of the national Get Britain Working strategy, will offer personalised one-to-one guidance from Employment Specialists, rapid job search and sustained in-work support. Participants will benefit from vocational profiling, skills development, access to training and help with workplace adjustments. We will work closely with employers to create inclusive opportunities, ensuring roles match individual strengths and aspirations.

Turn Connection into Recovery & Make Employment Central to That Journey!
This is a role that drives local integration and early momentum. As the Health and Community Integration Lead for Connect to Work, you’ll play a critical role in preparing the ground for delivery, building relationships, securing co-location, and raising awareness across health, voluntary and community systems.
During implementation, you’ll work with local stakeholders to position Connect to Work as part of the regions wider health and work agenda, recognising its role in improving population health and contributing to inclusive economic growth. That means understanding the existing landscape, mapping current provision, and working carefully to avoid duplication. You’ll help create clear, trusted referral routes into the service and ensure Connect to Work complements, rather than competes with, what’s already there.
We know it takes time to build the right relationships and earn confidence. This role is about creating the conditions for those connections to grow and helping the programme embed within the local system, not around it. As well as supporting referrals in, you’ll help establish the outward-facing links that allow Employment Specialists to connect participants with the support they need to stay in work and reconnect with their communities.
Location/travel: £29,000-£32,000
Location/travel: This contract will be carried out in: Blackburn, Darwin, Burnley, Pendle & Ribble Valley, therefore the ability to travel across this patch is essential.
What You’ll Do:
- Build relationships with health partners to generate referrals into Connect to Work
- Promote the health and wellbeing benefits of good work to clinical audiences - making the case for employment as a core component of recovery.
- Attend MDTs and co-locate with PCNs and community health teams
- Train and brief clinical staff on the benefits of employment and how to refer
- Share insight to improve integrated care and support continuous improvement
- Champion social value and inclusive employment at strategic forums and events
- Work closely with Employment Specialists and Occupational Inclusion Specialists to ensure customer journeys remain joined-up, person-centred, and reflective of clinical needs.
Your skills & experience:
- Experience working in or alongside primary care or community health
- Strong relationship-building and influencing skills
- A strategic mindset with hands-on delivery experience
- Passion for inclusion, co-production, and systems that serve people better
- Confidence to work independently across community and clinical settings
Desirable
- Background in public health, social work, occupational therapy, or employment support.
- Understanding of the Individual Placement and Support (IPS) model or supported employment approaches.
- Experience engaging with Integrated Care Boards (ICBs) or similar system-level health bodies.
