Overview
The Care Navigator plays a crucial role in supporting patients through their healthcare journeys. This position needs someone who can multi-task, think on their feet, follow protocols and work in a team environment. The ideal candidate will possess strong organisational skills and a compassionate approach to patient care, making them an essential part of our team.
Responsibilities
- Serve as the primary point of contact for patients when accessing the service.
- Main responsibilities will be call handling and managing the front reception desk.
- Maintain accurate records of patient interactions and progress within the office system.
- Collaborate with healthcare providers, social workers, and community resources to facilitate any patient queries.
- Requirements
- Previous experience in an office environment is preferred.
- Strong interpersonal skills with the ability to communicate effectively with diverse populations.
- Excellent organisational skills and attention to detail to manage multiple tasks efficiently.
- Empathy and a genuine desire to help others navigate their healthcare challenges.
- Proficiency in using office software and electronic health record systems is advantageous.

Pay: £12.21 per hour
Expected hours: 37.5 per week
