We are seeking a dedicated and compassionate Care Navigator to join our team. The ideal candidate will play a crucial role in guiding patients through their healthcare journey, ensuring they receive the necessary support and resources. This position requires excellent communication skills, strong organisational abilities, and a passion for helping others.
Responsibilities
- Assist patients in navigating the healthcare system, providing information on available services and resources.
- Coordinate appointments and follow-up care for patients, ensuring timely access to necessary treatments.
- Maintain accurate records of patient interactions and data entry into relevant systems.
- Communicate effectively with patients, families, and healthcare professionals to facilitate care.
- Provide administrative support, including clerical tasks and managing office correspondence.
- Use of electronic communications
- Make appointments
- Issue and request medication
- Scan and code in docman
- Recall patients
Skills
- Strong data entry skills with attention to detail for accurate record-keeping.
- Excellent organisational skills to manage multiple tasks effectively.
- Demonstrated phone etiquette for professional communication with patients and healthcare providers.
- Previous experience in administrative or clerical roles is preferred.
- Ability to type efficiently and accurately while managing various office tasks simultaneously.
