CARE NAVIGATOR - 35 Hours per week (Possibility of Job Share)
We are looking for a new Care Navigator to join our Patient Support Team. We are a busy but friendly Practice with a list size of around 5400 patients, situated in a purpose-built premises.
Our team includes: 4 Partners, 1 Salaried GP, 2 Advanced Practice Nurses, 1 Practice Nurse, 1 Nursing Associate, 2 Healthcare Assistants, 1 GP assistant, Management and Patient Support Team.
We are an established Training Practice for ST3s (GP Trainees), ST2s, Foundation Doctors and medical students (nurse, paramedics etc).
Working hours are as follows although applicant must be flexible as per needs of the Practice:
Monday – 8am – 5pm
Tuesday – 8am – 2pm
Wednesday – N/A
Thursday – 8am – 6pm
Friday – 8am – 6pm

Experience is preferred but not essential as training will be provided.
