Job Summary:
We are seeking a dedicated Care Coordinator to join our team in providing support to individuals in need of care services. The Care Coordinator will be responsible for organising and coordinating care services for clients, ensuring their needs are met effectively and efficiently.
Duties:
- Liaise with clients and their families to assess care needs and develop personalised care plans
- Coordinate care services, including arranging appointments and visits from healthcare professionals
- Monitor the delivery of care services to ensure they meet quality standards and client expectations
- Maintain accurate records of client information, care plans, and service delivery
- Provide administrative support, such as scheduling appointments and managing documentation
- Collaborate with healthcare professionals, social workers, and other stakeholders to ensure holistic care for clients
Skills:
- Excellent organisational skills with the ability to manage multiple tasks efficiently
- Strong communication skills, both verbal and written, for effective interaction with clients and colleagues
- Proficient in office software applications for data entry, record keeping, and report generation
- Empathetic and compassionate approach towards clients' needs and concerns
- Ability to work independently as well as part of a multidisciplinary team in a fast-paced environment
- This role role will require you to be able to drive and have access to own car with business insurance
