The Lantern Community is a unique and inclusive home for 53 residents (adults with diverse abilities) and welcomes over 20 day placements from the local area. Placed in 15 acres of beautiful Hampshire countryside, we provide meaningful opportunities through our 9 workshops, fostering independence, creativity, and a supportive community.
About the Role:
- Assist in maintaining records and completing administrative tasks as required, and ensure effective communication with House Co-ordinators.
- Assisting in the training, supervision, and daily management of co-workers (volunteers) within the house.
- Assist in organising and supporting regular one-on-one or group support sessions with the service users (Companions), as delegated by the House Coordinator.
- Assist in maintaining effective communication with the service users’ families, social workers, GPs, dentists, and other relevant clinicians as needed.
- Facilitate activities that encourage the development of the service users, regularly recording and reviewing their progress with the House Coordinator to help them reach their potential for independence.
- Planning, risk assessing and managing and carrying out activities to encourage the development of the service users, recording and reviewing progress, so that the service users (Companions) can fulfil their potential for independence.
- ·Assist in producing and implementing support and care plans by providing one-to-one support (or shared support when it is appropriate) and care for individuals offering a range of services which help promote independence, self-confidence and a positive self-image.
- Creating easy-read material for companions where needed to aid communication and implementing a range of communication methods.
About You:
- Proven experience in health care/social care settings, ideally in supporting adults with Learning Disabilities.
- A relevant professional qualification in Health and/or Adult Social Care (e.g., Level 2 or above in Health and Adult Social Care or equivalent).
- Proven literacy, numeracy, and IT skills.
- Ability to work independently and as part of a team.
- Strong aptitude for establishing effective working relationships within the community, including with service users' families, advocates, social workers, GPs, and other health professionals when needed.
- Problem-solving skills, along with the ability to give and receive constructive advice.
- Ability to plan and organise workload, maintain high-quality output, and meet set deadlines and targets.
- Capacity to adapt to changing situations and environments, demonstrating resilience.
- Ability to influence, motivate, and inspire others.
