About The Role
Are you passionate about making a real difference in people’s lives? This is an exciting opportunity to join our dedicated support team in Cornwall as a Referrals Coordinator within our New Horizons service in St Austell. You will play a vital role in helping individuals facing complex challenges access safe accommodation and holistic support.
Key Responsibilities:
- Support individuals to refer into the New Horizons service, primarily via phone and occasionally face-to-face in the community.
- Work with people experiencing complex needs such as substance misuse, mental ill-health, and domestic abuse.
- Provide trauma-informed, person-centred support and create tailored safety plans.
- Use creative communication and support planning approaches to meet individual needs.
- Conduct effective risk assessments and prioritise referrals based on urgency and risk.
- Build strong links with external services and providers to ensure comprehensive support.
- Navigate challenging conversations with empathy and professionalism.
- Work flexibly, including late shifts and some weekends, to meet client needs.
- Maintain accurate records and manage referrals using IT systems confidently.

This role is being offered on a full time, permanent basis, working 37 hours per week, over 5 days to include weekend working.

About The Candidate
To be successful in your application for the role of Coordinator - Referrals, you will have the essential skills and experience for a level 1 role (please see candidate information pack) and the following role specific skills and experience:
- Have an understanding of support needs and support planning.
- Have experience of risk assessing and risk management.
- Be a great communicator with the ability to build a rapport.
- Have accurate record keeping and good computer skills.
- Confident in lone working and be a good team player.
- Have a flexible approach to work.
- Be able to work under pressure and meet deadlines.
- Be resilient and adaptable.
- Have an open, friendly personality and the ability to maintain a positive attitude in challenging situations.
