We are looking to recruit 4 Reablement Co-ordinator on a permanent basis on a 32-hour contract who truly care and are committed to making a difference in an individual’s life.

Being a Reablement Coordinator means supporting people in their own homes for a short space of time to assist them to reach their full potential, this may include managing a variety of aspects of daily living skills, set clear and achievable goals to help people rebuild their former independence and confidence to sustain independent living at home and in their communities.
You will effectively work within the reablement team to deliver a high quality reablement service with the aim of promoting and sustaining people’s independence living in the community. Supervise staff / mentor personal development of Reablement workers
A driving licence is essential criteria for this community-based role.
Complete service users journeys well which coordinated and complies with organisational policies, procedures and Care Quality Commission regulated guidance. To ensure that the reablement service achieves the best quality outcomes for service users, their carers and families.
