Evening and night District Nursing team Central Manchester. He/she will work with patients, families, carers within Central Manchester, working with the integrated neighbourhood teams, to provide high quality specialist nursing care and support for patients in community settings.

He/she will exercise enhanced levels of judgement, discretion and decision making in clinical care and ensure all records and documentation are completed and relevant. He/she will demonstrate flexibility and adaptability to work safely in different environments. The post holder will have responsibility, for the management of a designated caseload, He/she will be responsible for the assessment, implementation and evaluation of programmes of care within the community setting without direct supervision.

The post holder will support the team leader with innovative clinical management and leadership of the team, mentoring the continued professional development of individual team members and assisting in the continued development of the District Nursing Service as a whole. The post holder will have delegated responsibility for the supervision and day to day management of junior team members.

As an experienced Registered General Nurse (RGN) you will be required to work as part of our Out of Hours District Nursing Service to provide complex nursing care to adults in their own home and in a variety of settings within the community.
You will work closely with the Out of Hours GP Service and the Integrated Neighbourhood teams to ensure that patient care is effectively coordinated, demonstrating a positive impact on our patients.
The Out Of Hours Service works from one Central base at Withington Community Hospital and covers patients within Central Manchester Locality. Shift patterns are from 5:30pm – 11:30pm and 10 30pm –8am and is 7 days per week.
To join us, you will need to demonstrate a keen interest in the field of Community Nursing, together with a genuine desire to develop both the service and yourself. Due to the nature of the role, it is essential that you possess a full driving license and have access to a car.
