What you’ll do:
· Lead and support a team of Support Workers in delivering person-centred, safe care.
· Implement and audit complex care plans, liaising with nurses and multidisciplinary colleagues.
· Act as on-call, Team Leader and ensuring swift response to incidents and safeguarding concerns
Who you are:
· NVQ 4/QCF 3 in Health and Social Care (or equivalent) with at least 2 years’ supervisory experience
· Skilled in Care Planning, Assessment and staff leadership
· Clear communicator, resilient, values-driven and CIW-aware
