Are you keen to work alongside the NHS to support people being discharged from hospital?
We are looking for a Support Worker to:
- Be responsible for developing and delivering a high-quality programme of support which promotes client choice and control.
- Address client support needs, enable them to achieve positive life changes, and ultimately empower them to lead more sustainable lifestyles.
- Work as part of a team to achieve control and quality outcomes.
