Job Overview
We are seeking a compassionate and dedicated Support Worker to join our team. The ideal candidate will have a passion for helping individuals with diverse needs, including those with developmental disabilities, Alzheimer’s, and other memory-related conditions. This role involves providing essential support in daily living activities, ensuring the well-being and comfort of our clients in various settings such as group homes, assisted living facilities, and private residences.
Duties
- Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and meal preparation.
- Provide patient care and support in accordance with individualized care plans.
- Monitor and document vital signs and changes in client conditions.
- Ensure a clean and safe environment by performing light housekeeping tasks.
- Assist clients with mobility using equipment such as Hoyer lifts when necessary.
- Engage clients in meaningful activities that promote social interaction and cognitive stimulation.
- Administer medications as prescribed and ensure compliance with HIPAA regulations.
- Provide companionship and emotional support to enhance the quality of life for clients.
- Collaborate with healthcare professionals to implement care plans effectively.
- Maintain accurate records of patient observations and report any concerns to supervisors.
Experience
Candidates should possess a combination of the following skills and experiences:
- Previous experience in home health, hospice care, or long-term care settings is preferred.
- Familiarity with working with individuals who have developmental disabilities or autism.
- Knowledge of first aid and CPR certification is highly desirable.
- Experience in patient monitoring, memory care, dementia care, or senior care is an asset.
- Ability to perform heavy lifting as required for client mobility assistance.
- Strong communication skills to effectively interact with clients, families, and team members.
- A background in social work or nursing is beneficial but not mandatory.
