The post holder will work with people under the care of the Specialist Community Forensic Team. You will provide support to individuals within secure hospitals & in the community setting. You will be able to build excellent therapeutic relationships with individuals working to enhance their discharge pathway.
You will be working with a team with a recovery focus to encourage the development of independence. You will be part of an intensive specialist multidisciplinary team and contribute to risk management under civil and forensic sections of the Mental Health Act ( 1983).
We are a small multi disciplinary team including Consultant Psychiatrist, Clinical Psychologist, Social Workers, Mental Health Nurses, Occupational therapist and Senior Recovery Worker.
Develop a rapport with service users within appropriate boundaries.
Encourage service users to be fully involved and engaged with their agreed care plan and to access appropriate services/ activities.
Implement agreed programmes of care collaboratively with service users and carers under the supervision of a named professional.
Help to identify early signs of relapse by monitoring the service user's progress, level of functioning and mental state and alert the appropriate staff involved in their care.
Take a role in evaluation of care provided.
Identify and support carer needs as required.
Take a role in health promotion including physical health needs.
Participate in and support involvement in social activity groups.
Maintain accurate service user records.

To have a working knowledge of the Mental Health Act

Receive supervision from agreed supervisor on a regular, planned basis, and work within the appraisal process
Access peer supervision and contribute to the development of others via attendance at regular multi disciplinary reflective practice meetings

To maintain accurate and up to date records of all contacts and interventions with service users
To undertake any other duties that maybe allocated by the Team Manager within the general scope of the post and appropriate to the grade.

The post holder should be comfortable in the use of computer technology, have basic keyboard skills and can use email.
