Assess and triage patients, including same day triage, and as appropriate provide treatment including prescribing medication following policy, local and national guidelines and local care pathways, or make necessary referrals to other members of the primary care team. Assess patients presenting with acute illness or for review of chronic disease conditions Undertake home visits for acute illness or chronic disease management Diagnose, plan implement and evaluate treatment and interventions and care for patients presenting with undifferentiated diagnoses. Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly Assess, diagnose, plan, implement and evaluate interventions and treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan Advise patients on general healthcare an promote self-management where appropriate, including signposting patients to other community or voluntary services.

Work with patients in order to support compliance and adherence to prescribed treatments Provide information and advice on prescribed and over-the-counter treatments Support patients to adopt health promotion strategies that promote health lifestyles, and apply principles of self-care Perform specialist health checks and reviews and perform investigatory procedures as required. Undertake the collection of pathological specimens including intravenous blood samples, swabs etc Support the delivery of anticipatory care plans and lead certain community services eg, monitoring blood pressure and diabetes risk of elderly patients living in supported housing Provide and alternative model to urgent and same-day GP home visits Communicate at all levels across organisations ensuring that an effective, patient-centred service is delivered. Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Offer paediatric related services and clinics Independent prescribing. We will consider applications from Paramedic Practitioners who have not yet achieved this they would be supported to do so in time. Other duties Work within the policies of the PCN and practices Promote client involvement in the management of the service Participate in regular supervision sessions and appraisals with your line manager, attending training and development activities and participate in meetings as required Maintain administration systems and workspace used in a clean and tidy condition Participate in policy development and data collection where appropriate Work flexibly to meet the needs of the patients Ensure mandatory training requirements remain up to date Recognise and work within own competence and professional code of conduct as regulated by the HCPC and College of Paramedics Keep up-to-date with current evidence-based best practice Support and participate in shared learning across the PCN and practices Support the mentoring of paramedic students Undertake any other duties appropriate to the grade and purpose of the job.
