The work will involve assessing, diagnosing and prescribing for those that have been through Memory Services who live predominantly in North Derbyshire area and this may cover South Derbyshire too. Due to the geographic area covered by the team there will be occasions where working in other localities will be required. As such you will need to have the ability to travel across a specified geographical area.
You will need to have recent relevant experience of dealing with older people with an organic illness, preferably in a community setting.
The posts will suit an enthusiastic, driven, motivated and flexible NMP with demonstrable experience of working in an independent prescribing practitioner role. Arrangements for Consultant support and prescribing supervision are in place. The post holder will be required to engage in CPD including a mix of self-directed learning and mentored practice supported by internally and externally taught training.
As a NMP, you will assess, diagnose and prescribe to patients who come through the memory assessment pathway.
