Your role and responsibilities

You will:

  • work with allocated customers to identify and deliver personalised, high-quality outcomes.
  • support the Home First team with assessments and reviews to ensure timely and effective care.
  • collaborate with social work teams, health professionals, and the voluntary sector to promote integrated care.
  • make a meaningful difference to the lives of Suffolk residents.
  • receive excellent support, supervision, and development opportunities.

You will need

  • Strong communication skills to engage effectively with customers, families, advocates, and professionals.
  • The energy and flexibility to respond to urgent work and contribute to team priorities.
  • Resilience and a positive attitude when working under pressure.

Home First provide a short term reablement care and support service to people living in their own homes.
