Following the exciting development of our Multispecialty Virtual Ward, we have a number of opportunities for roles within the service.
The Virtual Ward is an innovative service and the aim to is support people to receive care, support and interventions within their own home, this improves the quality of care for the individuals, reduces the length of time people stay in hospital, reduces the need for people to be admitted to hospital and ensure beds are available to treat the acute admissions.
This is an opportunity to gain experience and shape the development of the new service for the future. The role will involve working as part of a multidisciplinary team both within secondary care and the community providing care at home.

The successful applicant will provide support to the nursing team in providing nursing programmes to people being managed on virtual services pathways in the community. The post is an integral part of the multidisciplinary team which contributes to supporting patients to meet their nursing goals in their own home following an admission to hospital or a period of ill health. A full Driving license is essential for this role.

PRIMARY DUTIES & AREAS OF RESPONSIBILITY
Manage Care
- Work under the indirect supervision of the qualified nurse in collaboration with the multidisciplinary team (MDT)
- Implement patient care as delegated by the nurse in charge, reporting any change in a patient’s condition and offering immediate treatment if necessary
- Provide time limited programmes of care to patients
- To prevent unnecessary admission to hospital
- To facilitate early discharge from hospital
- To meet rehabilitation needs to promote and maintain independence
- Following a patient’s assessment by a member of the MDT including development of a programme of care (nursing and/or therapeutic) work to the care plan by using established skills and working within guidelines, protocols and competencies
- Contribute to the monitoring and review of programmes of care for patients with the multidisciplinary team and ensure a smooth transition of care to other providers e.g. District Nurse, Home Care
- Assist in assessing on-going nursing needs alongside qualified staff
- Assist in the delivery of nursing programmes with qualified staff members
- Undertake physical, clinical and functional treatment programmes in order to progress a patient’s pathway
- Undertake phlebotomy, cannulation and ECG
- Assess the patient’s response to programmes of care and report the findings to the MDT
- Use equipment appropriately and educate and advise carers in the use of and care of simple equipment.
- Identify risks to the patient or staff, reporting the information at the earliest opportunity to a qualified member
- Attend MDT to give information, opinion and feedback concerning the care of patients and report back to nursing colleagues as appropriate
- Demonstrate listening skills and deal sensitively with individuals who may be experiencing high levels of anxiety or aggression caused by pain or psychosocial factors
- Ensure agreed standards of care are achieved
- Support individuals in self-management of long-term conditions
