The Kaleidoscope Powys substance use service provides a range for services for anyone experiencing problems because of drug or alcohol use, including therapeutic interventions and prescribing medication. Overseeing the delivery of harm reduction services and advice, including the provision of Prenoxad training, Needle Syringe Provision and Dry Blood Spot Testing as well as prescribing medication such as Opiate Substitute medications,
We welcome individuals with their own lived experience of substance and alcohol use and/or mental health and from those who have supported others with lived experience.
Role Purpose:
The Court and Custody Worker role aims to improve the lives and wellbeing of people with multiple needs who require intensive support and may find it difficult to navigate or engage with services, revolve in and out of services or have barriers to accessing services.
The worker will use a trauma informed strength-based approach to assess a person and enable them to access the services they need that offer a range of evidence-based interventions to help people increase their resilience, gain confidence, acquire the personal and social assets they need to meet their goals, reduce risks and gain/maintain suitable, stable accommodation.
To work as part of an integrated team providing collaborative case management to those in contact with the criminal justice system. To provide an effective link with court and prison establishments, supporting those within the court process and entering and exiting prison custody. To provide holistic and structured recovery coordination and psychosocial interventions to service clients, their families and carers. Working to ensure effective delivery of recovery coordination within the full integrated service model.
The worker will develop a comprehensive data base that will track service users from court appearance and engagement with service through, their journey while in custody and their discharge. The person will. The worker will work with prison’s and develop care plans for the person’s release while in custody. This will include links and referrals to housing, treatment services and continuity of medication and CMHTs.
Duties and Responsibilities:
- Attend courts that cover Powys and seek ways to maintain a presence and contact with offenders who will appear at Merthyr Courts. To provided assessments and direction in relation to Community Treatment Requirement Orders.
- Actively establish and maintain effective working relationships with prison establishments and prison drug and alcohol recovery teams as a named single point of contact to facilitate seamless continuity of care in and out of prison establishments for service users, protecting recovery capital and engagement (including peer-led support).
- Where required provide prison in-reach to support positive community re-integration, including supporting abstinent individuals and linking with a wide range of community support organisations.
- To lead effective case management for allocated service users, including on-going processes such as risk assessments, risk management plans and safeguarding assessments and plans. This includes the effective use of an online case management system.
- Carry out triage/screening assessments and comprehensive assessments with service users, which may include drug screening and testing.
- Work in partnership to carry out joint assessments, key working and interventions to enable individual service user progression from engagement, through structured treatment to selfsustained recovery.
- 6. Identify and prevent potential harm to service users and others by following local and organisational safeguarding guidelines, including referral to and engagement with relevant safeguarding authorities.
- To work closely and collaboratively within the multi-disciplinary team across the service and externally, providing advice and reports/ information to managers and other colleagues, through attendance at reviews, team meetings, supervision and via telephone conversations.
- Develop and deliver quality programs and packages of work/ psychosocial interventions that meet local need across both group work and one to one formats for both male and female offenders.
- Maintain accurate, up to date records and work in accordance with organisational guidelines and procedures
- Undertake continuing professional development including participating in clinical supervision, performance reviews and attending training as/when required .
- Work in accordance with all relevant legislation, policies & procedures and guidelines – both internal and external. This includes Humankind’s clinical governance framework .

- To perform any other duties as deemed appropriate by management.
It is essential that the post-holder establishes and maintains good working relationships with clients and undertakes working with clients in a friendly and approachable manner. Failure in this area is likely to cause the post-holder to be inappropriate for the work involved. Although all staff are appointed to work in specific areas of Kaleidoscope it must be understood that all appointments are made to Kaleidoscope as a whole. In time of need, management are able to mobilise the staffing resource of Kaleidoscope to meet these needs.
This post has been identified as requiring the post holder to provide evidence of BBV immunisation status in order to carry out the role effectively. The role may require the post holder to undertake Dry Blood Spot Testing/ Substance Misuse Testing/ Working in Needle Exchange or is a clinical role and therefore Kaleidoscope wishes to ensure that staff are not at risk of infection or transmission of a disease that may affect their own or patient safety. Staff will therefore need to evidence their immunisation history, exemption or non-responder on commencement of employment or at routine intervals at the request of the Organisation
