Cornwall Partnership NHS Foundation Trust is leading a new vision for Community and Palliative End of Life Care across the county. Our aim is to support people nearing the end of life to experience a calm, peaceful, and dignified death in the place of their choice.

Cornwall has one of the highest national rates of extended hospital stays during the last 90 days of life. This highlights the need for a high-quality, community-based service that enables more people to receive the right care at home.

We are working to develop a service that is personalised, reduces unnecessary hospital admissions, and offers a single point of access for home-based care planning and support.

In partnership with the Macmillan Social Investment Fund and local organisations, we are enhancing services through a 24/7 Rapid Community Response and Care Service and a dedicated round-the-clock phone line for professionals and the public.

These developments will complement our Hospital@Home virtual service and voluntary sector partnerships, improving end of life support in the community.

The role of the progress co-ordinator is a countywide role working with Clinical Prioritisers, Palliative care team (Acute Hospital and Community Hospitals) and Hospital at home. The role supports the End of Life Pathway processing and managing the flow of patient referrals through the STRATA.

The progress co-ordinator will monitor the referrals on the single electronic referral system on a continual basis, triaging in partnership with the multi professional team.
The progress co-ordinator will be responsible for reviewing key blockages to patient flow, highlighting key issues to ward colleagues and Patient flow team by maintaining accurate and timely feedback to referrers, liaising with multi-agencies for the co-ordination of patient flow.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increase the numbers of people who use out services.
