We are looking for a Care Liaison Advisor to join our team, where you will play a vital role in our company's success. In this role, you will closely collaborate with hospital discharge teams and care homes to source the best care packages for our clients.

This position offers a unique opportunity to work towards quick and safe discharge plans whilst supporting families during challenging times.

Every day you will…

  • Source both short term and long-term care home packages of care for clients working towards a quick, safe and correct discharge plan for patients
  • Liaise with care homes, care agencies, senior health and social care staff, patients and their families
  • Meet and work with families in a sensitive and empathetic way
  • Provide and communicate updates and act as the link between discharge teams, local authority, clients and care providers to ensure the successful delivery of a high-quality service whilst ensuring discharge happens within set timescale
