Practice Care Coordinator

JOB PURPOSE
The core purpose of this role is to support the practice's commitment to deliver integrated, patient-centred care, responsive to the needs of our local population. The post holder will be a key member of the team, working collaboratively with colleagues to provide holistic and coordinated care to our patient community.
JOB SUMMARY
The post holder will be responsible for the co-ordination and delivery of patient centred care to ensure the achievement of key health and welfare objectives the practice. This pivotal role will involve working alongside other clinicians in the practice as part of a multi-disciplinary care team.
The post holder will focus on improving integrated care, particularly for patients with safeguarding concerns, those with long-term conditions and multi-morbidity, and individuals requiring a proactive care approach, including vulnerable patients. They will also play a crucial part in enhancing patient access and patient experience and deliver all-round care for patients. The role serves as a primary point of contact for patients and clinicians in coordinating care and will help to deal with a wide range of queries relating to the patients or other healthcare providers.
KEY RESPONSIBILITIES
- Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care in line with practice directed priorities, an example of this may include those with health inequalities.
- Support patients in managing their care needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information relating to their care and medication.
- Proactively recall patients for annual recalls and vaccinations such as diabetic reviews or flu vaccinations.
- Signpost patients effectively to appropriate community or local services.
- Serve as the contact point, advocate and informational resource for patients, care teams, family /caregivers and community resources where indicated, responding with empathy and respect.
- Assist with the identification of ‘high risk’ patients and keep a register of the teams’ workload.
- Support the practice to deliver on quality metrics such as QOF, KPIs and locally commissioned enhanced services.
- Contribute to practice capacity, access and improvement projects to meet the primary care network requirements.

The post holder will recognise the valuable contributions of other colleagues within the organisation and their role to patient care. They will demonstrate use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g. CCGs). The ability to work effectively as a member of a team is essential. The post holder must also recognise personal limitations and refer to more appropriate colleague(s) when necessary.
TRAINING AND DEVELOPMENT
- Complete all mandatory training.
- Actively participate in annual appraisals.

In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately. In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the network or individual practices as business organisations. All such information from any source is to be regarded as strictly confidential. Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.

HEALTH AND SAFETY / RISK MANAGEMENT
The post-holder must always comply with the Practice’s Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the Practice’s Incident Reporting System. The post-holder will comply with the Data Protection Act (1984) and the Access to Health Records Act (1990).
