This role forms part of the centralised team of Reablement Referral Coordinators and includes dedicated schedulers for the County, operating within Corserv Care to support STEPS and the wider cares services covering a 7-day service provision. The purpose of the role is to plan and organise the work of the Support Workers in the most effective manner whilst maintaining continuity of care for Service Users and supporting the Registered Manager and team with administration functions.

It is responsible for the scheduling and other administrative processes involved with taking on new customers, in a way that is target driven and maximises the efficient use of capacity It will assist the discharge Co-Ordinator’s with the scheduling, setting up new referrals on total mobile and all admin around this process that is involved with taking on new customers into the three Community services in each of the localities, in a way that is target driven and maximises the efficient use of capacity. 

The role involves preparing planned schedules of work but also adjusting schedules on a day-to-day basis in response to short notice sickness/absence, to accommodate new referrals and as a result of any other changing circumstances. Crucially, this team will ensure that all care visits are covered and that no missed visits occur. 

Under the leadership and management of the Registered Manager the Reablement Referral Coordinators, who will support the implementation of a new model of service delivery for Reablement covering Cornwall County, comprising of an initial up to 3-day assessment to assess for Reablement potential and a further continuation of up to 21 days to reach identified Reablement goals.

Your job will be:

Screen all initial new referrals into the “hub” to assess initial pathway and criteria of the service in times of the discharge co-ordinator on leave.  To support with setting up referrals when acceptances are high.  To scheduler across the county with your fellow scheduler looking at cross boundary sharing to ensure mileage is reduced, or referral are accepted in areas where they are higher. Sharing staff sensibly across teams and maximise referral acceptance. Covering each other when one is on leave, supporting with the discharge co-ordinators leave periods. 

 Work collaboratively with Lifeline to offer and assess further support using a TEC enabled approach. Close liaison with the Corserv Care’s, Hospital Discharge Co-ordinator, based at RCHT and UHP to ensure capacity is prioritised for medically fit patients requiring discharge to Corserv Care, through either the STEPS or Care & Support Teams to provide packages of care.

Close liaison with the Registered Managers to identify pending staff capacity to forward plan the creation of new care runs. Work closely with community-based Teams to discuss care needs of potential new customers to ensure these can be safely met by the team. 

Co-ordinate with community-based Teams to arrange follow on start dates, visits and care plan completion for new customers. Assess the potential to improve outcomes/lives gathering information from a range of sources, including adults, carers; and relevant others; other agencies, and other people within the person's wider support network. Promote independence by offering information and advice, signposting to other agencies as appropriate, and prompting contact with social and community resources. 

Promote and facilitate effective communication with families, carers and individuals and other agencies to ensure the views and wishes of individuals and carers are given appropriate consideration and are central to all decision making.
