JOB SUMMARY The PCN Projects Coordinator provides administrative support for vaccination programs, assists with project management activities, coordinates patient care across health services, maintains accurate records and documentation, engages with diverse community members, and ensures compliance with safeguarding and quality standards. RESPONSIBILITIES AND DUTIES TO INCLUDE: Vaccination Support - Schedule and manage vaccination appointments, waiting lists, and patient communications Maintain vaccination records, databases, and inventory management including cold storage monitoring Coordinate vaccination deliveries and communicate with practices and PCN teams Accompany vaccinator to patients' homes providing support for both practitioner and patient. Project Coordination - Assist with project planning, timelines, resource allocation, and stakeholder communication Maintain project documentation, monitor progress, and report on KPIs Coordinate team members, contractors, and resources to meet deadlines and budgets Patient Care Coordination - Navigate patients and carers across health and care services, liaising with GP practices and third-party organizations Manage patient appointments, recall programmes, and care review meetings Develop and monitor holistic care plans based on individual patient needs Administration and Documentation - Input and maintain accurate patient records in clinical systems while adhering to data protection legislation Handle patient documentation efficiently and share with relevant professionals Ensure complete case records following professional guidelines Community Engagement - Engage respectfully with diverse community members, translating complex health information into understandable terms Ensure patient and family views are incorporated into decision-making processes Develop links with local voluntary sector organizations and community groups Safeguarding and Quality - Understand and follow safeguarding protocols for vulnerable individuals Identify urgent needs and take appropriate action or escalate to team Use reports to proactively target patient groups and connect them with appropriate support services RECORD KEEPING, INFORMATION COLLECTION AND COMMUNICATION To ensure data recorded (by post holder and by those staff supervised by post holder) on all electronic systems conforms to necessary policies, processes and protocols. To be responsible for the accurate maintenance of clinical record keeping and the required communication of individual patient care packages for which the post holder has responsibility.
To be responsible for the accurate recording of information relating to the patients in receipt if care as determined by the PCNs operational policies or when directed by the Clinical Director. DEVELOPMENT AND SUPERVISION Participate in clinical audits and research as necessary to own work. To measure own personal goals ensuring they align to the wider PCN goals and strategic objectives. To assist service projects as and when required.
PROFESSIONAL AND PCN POLICIES The post holder must comply with all national, statutory, legislative, professional and local policy. The post holder should proactively contribute to improve local policy and any changes to improve service or protocols. To be responsible for the application in practice of the PCNs policies and procedures and the limit of the authority and responsibility the post holder has within these. To operate within the PCNs standing financial instructions and their application to financial and physical resources.
