Purpose of the Role: Contribute to the realisation of the outcomes for North East Lincolnshire, in particular to enable all people in NEL to feel safe and be safe, to have good health and wellbeing and to realise their potential through skills and learning. This will be achieved through the effective delivery of a range of support to young people and families in the locality, in order to improve their health and wellbeing and reduce the need for statutory intervention. Main Responsibilities: 1. Contribute to the delivery of a 0-19 years programme with specific focus on the delivery of the Health Visitor-led Healthy Child Programme 0-5 years 2.
Management and continuing responsibility for a designated caseload within an agreed caseload model for children aged 0-5 years and their families requiring community, universal, partnership and partnership plus interventions. 3. Prioritisation of conflicting demands 4. Work as part of a multi-disciplinary/agency team with allied health and social care professionals, individual clients, groups and communities.
5. Manage caseloads including vulnerable children and attend Child Protection case conferences. 6. Act as Nurse Prescriber (if qualified).
