Remote home working & Community Based – Office Use in Kings Lynn & Norwich
£14.55 per hour 37.25 hrs - Monday to Friday - 12 Months Contract
Officers hold responsibility for developing a person centred and co-produced Housing and Wellbeing Plan that facilitates the achievement of resolving housing related issues through a number of different toolkits, including mediation. The focus of the Early Intervention and Prevention Service requires close partnership working with key services and the ability to deliver focused, time sensitive and effective housing interventions. The successful candidate will have knowledge of housing and benefits, experience of working with young people and adults, lone working and managing a caseload.
Key Responsibilities:
- To achieve positive, successful housing outcomes
- To complete person-centered housing & wellbeing plans with individuals.
- To take the lead on accountably for performance -ensure target outcomes are achieved for clients.
- To oversee/support more complex referrals
- To support adults to achieve successful prevention of homelessness by keeping individuals in their home or finding alternate accommodation -working both remotely and within the community/housing options team
- To provide in-person mediation between family members, friends, and landlords, where appropriate, to resolve barriers contributing to the risk of homelessness.
- To source properties through active landlord engagement in the privately rented sector
- To support individuals to achieve up to 8-month sustainment of their accommodation (over an 11-12-month intervention, including 3 months of intensive support with to prevent homelessness and up to 8 months of monitoring)

This is a community based service, therefore a driving license and access to a vehicle are essential for this role
