Community Assessment Service:
A team designed to relieve the rising pressure on the NHS and Social Care across Wales. A team that has grown from strength to strength over the past 5 years.
You would part of the Community Assessment Service – Your job is to support individuals from hospital/institutionalised care, back home into the community. During this you will assess the needs of the individual over a period of 6 weeks then complete a person-centred assessment/care plan to suit their needs. You will work closely as part of a team of social workers, hospitals, external support services, to ensure that the service users final care package Is best suited to them.
£12.43 for unqualified QCF
£13.20 for Level 3 QCF.

Rolling rota – 3 days one week 4 days the next, Every other weekend off.
10 hour shifts – mornings and evenings.
Area’s include – Llanelli and surrounding areas.
