Summary
As a Care Coordinator at Serenity Always Ltd, you will play a vital role in managing and coordinating care services for our clients. This position is essential in ensuring that our clients receive the highest standard of care tailored to their individual needs.
Responsibilities
- Coordinate and oversee the delivery of care services to clients in their homes.
- Develop personalized care plans based on individual client needs and preferences.
- Communicate effectively with clients, families, and care staff to ensure seamless service delivery.
- Monitor client progress and adjust care plans as necessary to meet changing needs.
- Maintain accurate records of client information and service delivery.
- Assist with recruitment, training, and supervision of care staff.
Requirements
- Previous experience in a care coordination or similar role is preferred.
- Strong organizational skills with the ability to manage multiple tasks effectively.
- Excellent communication skills for interacting with clients and team members.
