The Role - Youth Practitioner/Keyworker

SCH is a growing company based in South London, we are a dedicated organisation committed to providing support and housing solutions for vulnerable young people in care. We support and prepare young people to live independently and reach their full potential. You will be providing support to young people between the ages of 16 to 25 years old. You are required to work within a semi-independent setting where young people share accommodation with 24/7 hours support and supervision. We aim to provide a secure environment that will support young people and to ensure they receive the highest standard of care appropriate to their individual needs.

Applicants will need to be dedicated and enthusiastic in providing good outcomes for young people in all aspects of their lives and have the ability to communicate effectively with young people. As a Youth Practitioner/Key Worker, you will play a crucial role in enhancing the lives of young people. You will be a role model, a mentor and a guiding figure in their journey to independence.

Duties & Responsibilities
- To assist young people with support around education, health, budgeting, meal preparation, shopping, activities and developing independence skills.
- To complete daily logs, support plans, risk assessments and monthly/weekly reports and ensure these contain accurate and up-to-date information to a high standard.
- Conduct regular key work sessions and produce reports on young people's progress, concerns or changes etc.
- Attend Admission, Planning Meetings, LAC Reviews, Education Meetings and any other relevant meetings and ensure all relevant paperwork is completed.
- Active as a positive role model to young people, offer appropriate advice, guidance and assistance.
- Support young people to build a trusting relationship, whilst promoting their physical, emotional, cultural and social well being.
- To carry out Health & Safety checks, record and report repairs works to the property.
- Ability to work in partnership with young people, parents, Social Workers and other professionals.
- Ability to engage and relate to young people.
- Ability to promote safe and consistent boundaries with young people.
- Any other duties/task that may be required by your Team Leaders or Managers.
- Attend training events, team meetings and structured supervision sessions.

Qualifications & Experience
- Candidates must have NVQ Level 3 or above in Health and Social Care (Children & Young People) or equivalent qualification.
- Experience working with young people with high risk of CSE, CCE and complex needs.
- Relevant experience of managing challenging behaviour, mental health experience, substance misuse, emotional behavioural and working with young people in caring or a similar setting.
