Main Duties and Responsibilities To support the Discharge Team in facilitating timely discharges for complex patients Support the maintenance of Advantis ward to ensure it reflects the current discharge plans . Attend Whiteboard rounds and ward rounds, identify actions required and actively manage agreed discharge plans for patients with predicted discharge dates within the next 3 days Maintain whiteboards/Advantis ward information to ensure it is up to date Assist in the completion of discharge check lists to ensure accurate discharge planning is evident, under the direction and supervision of the registered nursing staff Transfer patients to the discharge lounge if required Liaise with the Registered Nurse in charge / shift coordinator and resolve issues preventing prompt patient transfer Under the direction of the Registered Nurse only, ensure plans are agreed with patient and carers for discharge and ensure they are fully aware of any requirements to assist discharge Utilise IT systems to secure and record timely and accurate data relevant to the role and function The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately. If successful this vacancy will require a Standard DBS check at a cost of £26.90. This cost will be deducted from your first Trust salary In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
