The department of Haematology is looking to recruit an enthusiastic and well-motivated Thrombotic thrombocytopenic purpura (TTP) MDT Co-coordinator, who will support the successful set up and delivery of the TTP Specialist Centre (STTPC) which sits within a defined geographical catchment area with a network of referring hospitals hosted by Sheffield Teaching Hospitals NHS Foundation Trust. This role sits within the Haematology Department.

This role is integral to the STTPC, which will co-ordinate, support and promote a system wide networked approach to delivery of TTP services. The post holder will be required to work closely with the Clinical Teams to co-ordinate the collection and recording of mandatory data from partner organisations and prepare for, record and disseminate outputs from monthly network MDT meeting.

You will require experience of working in a customer/patient-focused environment, excellent communication and IT skills, along with good organisation and interpersonal skills and have the necessary confidence and competence to work with a wide range of professional staff, internal departments and external organisations.

They will need to be able to work as part of a multi-disciplinary team, as well as demonstrating the ability to work independently as required.

. Coordinate the referral process, record and disseminate outcomes from monthly network MDT meeting.

. To develop excellent working relationships and clear communication channels with all staff, both clinical and administrative, across the Network.

. To improve communications across the referral/ treatment pathway to promote a smooth passage for patients

. To support the clinical teams in the coordination of region/network wide education programmes.

. Provide administrative facilitation of the STTPC quarterly meeting, including preparing and circulating agendas and minutes.

. To co-ordinate and maintain robust TTP data collection and validation mechanisms

. To be responsible for the efficient collection and data entry onto the patient tracking database for Outcomes Monitoring Dataset

. To locate all missing data and ensure accuracy when inputting to the TTP database, working with the Clinical Nurse Specialists, Consultant Physicians and the wider TTP network as needed.

. To collate and report data as required, obtaining data from a variety of sources including patient records.

. To support & maintain research registries & databases for TTP

. To be responsible for the provision of timely and accurate information to management and other staff and ensure the production of reports and analysis when required.

. To be responsible for the undertaking of data collection, analysis and reporting and to participate in audits as required.
