We are currently recruiting a Transition Support Worker at Hertford Regional College to support both the transition of learners into Inclusive Learning and Mainstream HNF provision, and to provide additional assistance once learners have commenced their study at HRC

About the Role
We are seeking a dedicated and organised individual to support the transition of learners into both Inclusive Learning and Mainstream High Needs Funding (HNF) provision at HRC. This role involves coordinating all transition arrangements, including pre-entry assessments, and providing ongoing support to students once they begin their studies. The successful candidate will act as a key point of contact for schools, parents, and carers, delivering tailored outreach and transition services for learners from MLD (Moderate Learning Difficulties) schools. A key responsibility is building and maintaining links with local schools, identifying students in Year 13 who may benefit from early referral, and facilitating their transition through a bespoke Taster Programme. About You
You will have a minimum of Level 2 qualifications in English and Maths, ideally along with a Level 3 qualification in a relevant subject but this is not essential. You’ll bring experience of working with young people with Moderate Learning Difficulties (MLD), as well as working closely with parents, carers, and external support agencies such as Services for Young People (SfYP) and the Hertfordshire 0–25 Together Service. Confident in using Microsoft applications, you will be well-organised, proactive, and committed to supporting successful transitions for learners. Experience in either the school or further education (FE) sector is desirable, along with a strong understanding of inclusive education and individual learner needs.
