· Provide high-quality, person-centred care and support in line with individual care plans.
· Lead and support a team of support workers in the delivery of care services.
· Assist in the development and regular reviews of care and support plans.
· Act as a role model and provide hands-on support, especially in complex care situations.
· Carry out mentoring, and on-the-job coaching of care staff.
· Promote and safeguard the welfare of service users at all times.
· Administer medication following policies and procedures.
· Liaise with families, health professionals, and external agencies as appropriate.
· Keep accurate, up-to-date records and ensure documentation is completed to a high standard.
· Respond to and manage incidents and report concerns promptly to the manager.
· Carry out on call duties as and when required, delegated by the manager.

Pay: £13.73 per hour
Expected hours: 37.5 per week
Ability to commute/relocate:
- Ellesmere Port CH66: reliably commute or plan to relocate before starting work (preferred)
Experience:
- Senior care: 2 years (preferred)
