A vacancy has arisen within the Early Intervention and Prevention service for a Community Care Assistant in the Access Team working 36 hours per week to cover Maternity leave for up to 12 months. The team covers North and South Perthshire, as well as Perth City and so you must be flexible and able to respond to need as required. Due to the nature of the work, a driving licence is essential.
The Early Intervention and Prevention Service provides a first point of contact for adults and carers who wish to access Social Work Services across Perth and Kinross. We support Adults age 16+ across all client groups. This work is supported by a range of partners including GP's and 3rd sector organisations. Our aim is to enable people to live more independently in their communities - providing care and support at the right time, in the right place, when it is most needed.
The successful applicant will assist in maintaining vulnerable people in the community by providing a range of social care and care management tasks. These include carrying out Outcome Focussed Assessments, the negotiation of flexible packages of care, and the monitoring and reviewing of individual care packages delivered via SDS (Self Directed Support).
You will have the opportunity to participate in on the job training and your continuous development will be encouraged and supported throughout your career with Perth & Kinross Council.
This is an excellent opportunity to join a well established, highly skilled team, with excellent training, supervision and development opportunities provided. Benefits also include flexible working scheme, modern working practices, generous annual leave entitlement and pension scheme.
For purposes of connection and collaboration with colleagues and with the community we serve, our full-time employees work on site for a minimum of 2 days per week with the arrangements for part time employees being agreed on a basis which will ensure that they benefit to an equivalent degree.
