Job Title: Health Care Assistant/Phlebotomist
Responsible to: Partners / GPs / Business Manager/Practice Manager
Job Purpose: To provide a high-quality care service based on the needs of the Practice population which includes treatment, screening, preventative care, and patient education. Assisting GPs/PN/NA
Duties and Responsibilities:
Clinical:
1. To manage patients with chronic disease, running specific clinic sessions and supporting the maintenance of robust call and recall systems
2. Venepuncture and other skill-based tasks
3. Home Visits to Housebound Patients to Manage Long Term Conditions
4. Assist with Minor Surgery – Aseptic Technique
5. Looking at the “Unmet Needs” of all the patients with LTC including Housebound
6. NHS Health Checks
7. Function as chaperone when required
8. Undertake health promotion to help educate patients
9. Hypertension Clinics
10. Weighing & Measuring Patients
11. Undertake Opportunistic Screening
12. Update Care Plans and inform all people involved in the patients care
13. Health Promotion – Provide individual advice on Smoking Cessation, Health Eating & Refer to in-house staff – H&WBC, Social Prescriber
14. Develop & Maintain Health Promotion Material with the Practice/Notice Boards
15. Refer to other agencies as appropriate
16. Infection Control – Apply Infection Control Measures according to local and national guidelines. Support patients and colleagues in adopting sound infection control measures.
17. Safeguarding – Adults/Children
18. Competent in Anaphylaxis and Resuscitation Techniques
19. Assisting with Flu Clinics
20. 24 Hours ECGs after completing required training
Administration
To maintain accurate patient records and enter onto the computer using agreed Snomed Codes
Ensure accurate completion of all necessary documentation associated with patient health care
To collect data and participate in regular Practice audit and research projects
To contribute to the preparation of any Practice development plans
Mental Health – Recognises and is aware of appropriate referral mechanisms for family violence, substance a use and addictive behaviours
General
To maintain a safe and therapeutic environment for all staff, patients, carers, and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc) are implemented and adhered to
To always promote a professional approach by attire and attitude to patients and staff both in and out of the Practice
To maintain own continuous professional development and ensure PDP kept up to date in line with the requirements of the Practice
Attend and participate in staff and Practice meetings and in-house clinical training sessions
Any other reasonable duties that may be required from time-to-time
