Duties
- Assist patients with daily living activities such as bathing, dressing, and grooming.

- Communicate effectively with patients, their families, and healthcare team members to ensure continuity of care.
- Monitor and record patient progress, reporting any changes or concerns to the supervising nurse or healthcare professional.
- Provide companionship and emotional support to patients, fostering a positive environment.
- Maintain cleanliness and organisation within the care setting, ensuring a safe environment for all.
- Drive to patients' homes as required for home care visits, ensuring timely arrival and adherence to schedules.
- Utilise IT systems for documentation and communication purposes as necessary.
Requirements
- Proficiency in English is essential for effective communication with patients and team members.
- Previous experience in home care or a care home setting is highly desirable.
- Familiarity with creating and following care plans is an advantage.
- A valid driving licence is preferred for roles involving travel to patient locations.
- Strong interpersonal skills with the ability to empathise and connect with patients from diverse backgrounds.
- Basic IT skills for documentation and communication tasks are beneficial.
- A commitment to maintaining patient confidentiality and adhering to health and safety regulations.
